Virtual Host Config Issues -- Part 1

Well, I'm stuck again, and this time I'm stuck at
both ends of the stick. I'll separate them into
two posts.

I am trying to setup these two virtual domains:

www.ideastream.com
catapult.ideastream.com

I'm using www.everydns.net for DNS services. I've gotten
www.ideastream.com to resolve, at least, although
http://dnsreport.com says there's a slight misconfiguration (but I'll get to that later.)

The second virtual domain doesn't resolve at all.
dnsreport.com shows lots of red when I put that URL in.

I've tried to follow the examples for A and CNAME and all, but I've not been able to puzzle it all out yet. Here's a tally of the setup I've got:

Host: catapult.ideastream.com
Type: CNAME
Value: ideastream.com
TTL: 3600

Host: ideastream.com
Type: A
Value: 65.254.63.231
TTL: 3600

Host: www.ideastream.com
Type: CNAME
Value: ideastream.com
TTL: 3600

Where have I gone astray?
